Job Title: Case Management Specialist
Status: Full-Time
Job Summary: The Community Based Case Management (CBCM) Case Management Specialist shall be a member the multi-professional team responsible for ensuring all consumers are provided with a complete an integrated consumer-centered intake assessment. The assessment shall include, but not be limited to, consumer strengths, preferences, abilities, needs, and historical data regarding the consumer’s family, social support, cultural influences and medical status including psychiatric illness, substance abuse legal; employment, education, abuse and neglect issues, crisis planning and activities of daily living.
Minimum Education: A Bachelor’s degree with a minimum of twelve (12) semester credit ours in courses such as counseling, criminal justice, human services, psychology, social work, social welfare, sociology, or other behavioral science AND 1-1/2 years of specialized experience. An exception maybe made for the above-mentioned educational requirements, on an individual case by case basis, if a candidate meets the following combination of required minimum education & experience: 1) HS diploma or GED or AA PLUS CSAC or 2) HS diploma or GED or AA PLUS either a) at least twelve (12) hours earned college credit hours in human services/social services subject OR b) at least forty (40) hours of continuing education in human/social services subject. The continuing education may consist of seminars, conferences, adult education courses, certification coursework, employer training or similar educational activities. 3) Required Experience: The equivalent of two (2) years Full-time work experience providing direct behavioral health services to severe & persistently mentally ill (SPMI) consumers & demonstrated working knowledge of cultural norms, values and issues related to cultural diversity in Hawaii and demonstrated working knowledge of the health/human services system and community resources of Hawaii. 4) Required References: Three verified professional references attesting to the individual’s skills, ability & competency to meet the position duties and responsibilities.
Minimum Field-of-Expertise: Strong clinical skills and experience demonstrating competence in problem-solving, critical thinking, collaboration, and clinical competencies in providing education and training to staff members.
Required Experience: Must meet minimum educational and/or experience as noted above; interest and knowledge of current trends and new developments in the mental health field for the adult target population.
